Disease Management

Monitoring, lifestyle planning, and education for those with high-risk conditions

At Intermountain Health, we offer free disease management programs for members living with Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and Diabetes. It’s our belief that patient education and improved self-management can help monitor and improve treatment plans to enhance your long-term health.

Resources for Enrolled Members

Enrolled members in the disease management program are offered communication with a Registered Nurse or other health professional(s) to assist them with their conditions and how to appropriately manage their symptoms more effectively. They are given a better awareness and understanding of their condition by addressing lifestyle changes, current medication information, and even provide support to help prevent complications, exacerbation, or the development of other health problems not associated with their chronic condition. All members receive educational materials about their disease.

  • Appropriate utilization of services

How to Join

Members are identified as potentially having CHF, COPD, and diabetes through high-risk assessments, referrals from utilization management, case management, or by their primary care provider or specialist. Members may also self-refer by contacting their provider directly. Participation in the program is voluntary and there is no additional cost to patients.
